MHI-PAC Prior Approval Form
Thank you for your interest in supporting the industry’s efforts on Capitol Hill. To comply with federal campaign rules, trade associations with corporate members (like MHI) must obtain approval from its member companies before it can educate and solicit their employees regarding MHI-PAC. MHI members like you give this authorization by completing the form below, known as a prior approval form. If you are your company’s point of contact with MHI, you may complete this form that MHI-PAC uses for internal purposes only to comply with Federal Election Commission (FEC) rules. Submitting this approval form does not obligate you or your employees to contribute to MHI-PAC. It merely authorizes MHI-PAC to communicate with and solicit contributions from you, as well as the designated executive and administrative staff in your company, if you choose, in order to advance the industry’s advocacy. Your authorization below indicates you understand that participation in MHI-PAC is voluntary, and individuals being solicited have the right to refuse to contribute without fear of reprisal. It also indicates that to the best of your knowledge your company has not authorized any other national trade association PAC to solicit its employees during the calendar years designated. Questions? Please contact MHI’s Gay Westbrook at (703) 229-6212 or gwestbrook@mfghome.org. Please help us reduce administrative costs by designating all six years below
